California's State Auditor issued 1,400 recommendations across 173 reports; agencies implemented 1,146 — 82 percent, and the division reconciles exactly. The remaining 254 equal 1,400 − 1,146 to the digit: every unimplemented recommendation has been open past the one-year statutory clock. Meanwhile the high-risk list shows what persistence looks like: information security, designated in 2013, still scores 1.6 of 4.0 against the state's own 2.0 baseline — thirteen years on.

The situation

A closure rate that reconciles, and a risk list that doesn't move.

A legislative budget subcommittee uses the audit office's own tracking to inform appropriations: recommendations are tracked on a fixed statutory schedule (60 days, 6 months, 1 year), and agencies unimplemented after a year must explain themselves to the Legislature.

We entered the office's published counts and its December 2025 high-risk assessment, and had the report reconcile the closure rate, verify the statutory-tail identity, and compute years-on-list for each persistent issue.
